Hospital-to-Home™ by Remnant Home Care is a premium, non-medical, white-glove concierge transition service designed to help manage those details.

Safe Landing™ focuses on preparing for the client's arrival, coordinating the transition home, and helping the client become comfortably settled once they arrive.

Before You Arrive Home

The Goal: Home ready before you arrive.

Depending upon the client's individualized transition plan, Safe Landing™ pre-arrival services may include:

  • Pre-arrival home readiness check
  • Arriving at the home before discharge when requested
  • Preparing the client's primary recovery and living areas
  • Light home reset and organization
  • Coordinating additional cleaning services when requested or needed
  • Freshening or preparing bed linens and the client's resting area when requested
  • Grocery shopping
  • Stocking the refrigerator with requested foods, beverages, and essentials
  • Purchasing requested household necessities
  • Meal preparation or meal coordination
  • Picking up prescriptions
  • Picking up requested recovery or comfort supplies
  • Making commonly needed items conveniently accessible
  • Coordinating other approved concierge requests before the client's arrival

Client is responsible for the cost of groceries, prescriptions, supplies, third-party services, and other purchases made or arranged on their behalf.

Arriving Home

The Focus: Settled, supported, at ease.

Once home, the focus shifts from transportation to settling in. Support may include:

  • Getting the client comfortably settled
  • Bringing belongings into the home
  • Making sure requested food and beverages are available
  • Preparing a light meal or refreshment
  • Organizing commonly used personal items
  • Making requested essentials easy to access
  • Organizing discharge paperwork in an accessible location
  • Medication reminders
  • Helping coordinate prescriptions and pharmacy needs
  • Confirming upcoming appointments
  • Coordinating transportation for follow-up appointments
  • Technology assistance when needed
  • Communicating appropriate updates to designated family members
  • Companionship and reassuring presence
  • Approved concierge errands
  • Other practical non-medical transition needs identified in the client's plan

Client is responsible for the cost of groceries, prescriptions, supplies, and other purchases made or arranged on their behalf.

Pre-arrival Safe Landing™ services require authorization from the client or authorized representative and appropriate access to the residence. Remnant Home Care team members do not enter a client's home without prior authorization.

Discharge-Day Concierge Support

Personalized support surrounding the actual discharge.

Discharge day itself can be one of the most hectic moments of the transition. Hospital-to-Home™ may include hands-on concierge coordination to help manage the practical details as they unfold.

Remnant Home Care does not interpret medical discharge instructions or provide clinical advice. Any clinical questions are referred to the client's appropriate healthcare professional.

Depending upon the transition plan, discharge-day services may include:

  • Discharge-day coordination
  • Coordinating transportation home
  • Accompanying the client from the hospital to home when arranged
  • Assistance managing personal belongings during the transition
  • Prescription pickup
  • Grocery or essential-item pickup
  • Helping coordinate communication between the client and designated family members
  • Helping organize discharge paperwork for the client's reference
  • Assisting with practical transition details as they arise

Client is responsible for the cost of prescriptions, groceries, and other purchases made or arranged on their behalf.

A Thoughtful Concierge Touch

The Remnant Welcome Home Gift Basket

Hospital-to-Home™ should feel different from ordinary home care. When included in the client's selected Hospital-to-Home™ experience, Remnant may prepare a thoughtful Welcome Home Gift Basket for the client's arrival.

The basket may be personalized when appropriate and may include approved comfort items selected for the individual client.

We want the client to feel expected, welcomed, and cared for when they arrive home.

May include comfort items such as:

  • Bottled water or preferred beverages
  • Light snacks or requested foods
  • Comfort items
  • Personal-care essentials
  • A handwritten welcome-home card
  • Other appropriate items selected for the individual client

The Welcome Home Gift Basket is a thoughtful concierge amenity. Items are not intended to have therapeutic or medical benefits. Complimentary basket items are provided as identified by Remnant Home Care; additional or specialty requests may be the client's responsibility.

Within the First 48 Hours After Discharge

Designed for clients who need concentrated white-glove support surrounding discharge and the immediate transition home.

Hours may be used according to the client's transition plan for pre-arrival preparation, Safe Landing™, discharge-day assistance, settling into the home, errands, meals, groceries, prescriptions, transportation, family communication, and other approved concierge needs.

During the First 7 Days After Discharge

Designed for clients and families who want more comprehensive support throughout the first week following discharge.

Support may include Safe Landing™, groceries, meals, errands, prescriptions, transportation, appointment coordination, family communication, companionship, home organization, and other approved concierge services.

Because hospital discharge dates and times can change, services are coordinated based upon staff availability and the client's evolving discharge plan. Need additional personal-care support?

Hospital-to-Home™ is designed as a premium non-medical concierge transition service. If the transition planning process identifies that the client requires hands-on assistance with bathing, dressing, toileting, transfers, or other activities of daily living, Remnant Home Care will explain the appropriate personal-care service pathway.

An RN assessment and individualized plan of care are required before applicable hands-on personal-care services begin. Remnant Home Care can help coordinate the appropriate assessment and care plan when hands-on assistance is needed.

About Hospital-to-Home™ scope of service

Hospital-to-Home™ is a non-medical concierge transition service. Remnant Home Care does not use Hospital-to-Home™ to provide skilled nursing, medical assessment, diagnosis, medication administration, medical treatment, rehabilitation, physical therapy, occupational therapy, clinical interpretation of discharge instructions, or medical decision-making.

Remnant caregivers provide medication reminders only — clients self-administer their own medications. Remnant team members assist with non-clinical coordination, organization, reminders, observation, communication, and approved concierge support within the client's service plan.

Any medical concerns should be directed to the client's appropriate healthcare professional or emergency services when indicated. Hospital-to-Home™ is not an emergency-response or medical-monitoring service.

Client purchases and third-party expenses

Hospital-to-Home™ service fees cover Remnant Home Care's concierge time and services. They do not include the cost of groceries, prescriptions, household supplies, meals, personal items, recovery supplies, transportation charges, third-party cleaning services, or other products and services purchased or arranged on the client's behalf unless Remnant Home Care specifically states otherwise in writing.

When Remnant Home Care shops for, picks up, orders, or coordinates items for a client, the client is responsible for the cost of those purchases and any applicable third-party fees. Client purchases may be funded through the client's established Remnant Care Bank™ or another payment arrangement approved by Remnant Home Care.

Third-party providers—such as professional housekeeping, transportation services, or meal delivery—are separate from Remnant Home Care, and their charges are the client's responsibility unless specifically stated otherwise in writing.
