About our scope

All Remnant Home Care services are non-medical. We do not provide skilled nursing, medical assessment, medication administration, rehabilitation, physical therapy, or clinical care of any kind. Remnant caregivers provide medication reminders only — clients self-administer their own medications. Clinical questions are always referred to the appropriate healthcare professional.

Call or email Remnant Home Care. Share a brief overview of the patient's situation and care needs. No formal paperwork required to get the conversation started.

We identify the right service fit

We'll discuss the patient's needs and help identify which Remnant service — Traditional Home Care, Concierge Home Care, Senior Concierge Services, or Hospital-to-Home™ — is the right fit. We then connect with the client or authorized family member.

A care or transition plan is created

A personalized plan is built around the client's specific situation — whether that's an immediate discharge transition, ongoing scheduled care, or flexible concierge support.

Step 04

Services begin

Our team goes to work. For Hospital-to-Home™, that may begin before the patient leaves the hospital. For ongoing care, we schedule and begin as agreed upon with the client and family.

Scheduling as early as possible is recommended.

Because hospital discharge dates can change, early contact allows us to build a transition plan and confirm availability before the discharge date is finalized. Same-day or next-day requests are handled based on staff availability.

Signature Component

Safe Landing™

Safe Landing™ is Remnant's signature pre-arrival home preparation and settling-in service — the component of Hospital-to-Home™ that ensures home is ready before the patient arrives.

For discharge planners and social workers, Safe Landing™ means you can send a patient home knowing a professional team has already arrived to prepare the space, stock the refrigerator, pick up prescriptions, and ensure the environment is organized and ready — before the patient walks through the door.

Safe Landing™ pre-arrival services may include:

  • Pre-arrival home readiness check
  • Preparing the client's primary recovery and living areas
  • Grocery shopping and refrigerator stocking
  • Purchasing requested household necessities
  • Meal preparation or coordination
  • Prescription pickup
  • Picking up requested recovery or comfort supplies
  • Making commonly needed items conveniently accessible

Client is responsible for the cost of groceries, prescriptions, supplies, and other purchases made on their behalf.
