A premium, package-based concierge program designed for the critical days following hospital discharge. Hospital to Home™ handles the practical details of coming home — so the client can focus entirely on recovery.
Important Distinction
Hospital to Home™ is a time-limited transition program, not an ongoing hourly care tier. After the transition period, clients may continue with Traditional Home Care, Concierge Home Care, or Senior Concierge Services based on their needs.
